Services Provided
West Phoenix Mobile Lab Services provides mobile laboratory specimen collection services only. Our services include, but are not limited to, blood draws, specimen collection, and specimen pickup and delivery to certified laboratories.
We do not:
- Perform laboratory testing
- Analyze specimens
- Diagnose medical conditions
- Provide medical advice
- Access or release test results
All laboratory testing and results are handled exclusively by the ordering healthcare provider or certified laboratory.
Appointments & Cancellations
Appointments may be scheduled online, by email, or by phone. Same-day appointments may be available based on availability.
Please notify us as soon as possible if you need to cancel or reschedule. Failure to provide reasonable notice may result in a service fee.
Payments & Fees
We offer cash-pay services for individuals who choose not to use insurance.
- Prices are subject to change
- Payment is due at the time of service unless otherwise agreed
- Additional fees may apply for STAT collections or specialty services
West Phoenix Mobile Lab Services does not bill insurance directly.
Accessibility & Accommodations
We are committed to providing accessible and inclusive services in accordance with the Americans with Disabilities Act (ADA) and applicable Arizona state laws.

West Phoenix Mobile Lab Services is not liable for:
- Laboratory testing errors
- Delayed or lost results caused by third-party laboratories
- Medical decisions made based on test results
- Circumstances beyond our control, including environmental or safety conditions
Our liability is limited to the cost of the service provided, to the extent permitted by law.
Changes to Services or Terms
We reserve the right to update or modify these Terms & Conditions at any time. Changes will be effective upon posting to our website.
Governing Law
These Terms & Conditions are governed by the laws of the State of Arizona, without regard to conflict-of-law principles.
