Refund Policy

At West Phoenix Mobile Laboratory Services LLC, we are committed to providing reliable, professional, and convenient mobile specimen collection services. Please review this Refund Policy carefully before booking an appointment.

Service Fees

All fees charged by West Phoenix Mobile Laboratory Services LLC are for mobile phlebotomy and specimen collection services only. Laboratory testing fees are billed separately by the partnering laboratory and are not controlled, processed, or refunded by. By booking an appointment with West Phoenix Mobile Laboratory Services LLC, you acknowledge that you have read, understood, and agree to this Refund Policy.

West Phoenix Mobile Laboratory Services LLC does not perform laboratory testing and does not have access to test results.

Cancellations & Rescheduling

 - Appointments canceled or rescheduled at least 24 hours prior to the scheduled appointment time are eligible for a full refund of the mobile service fee or may be rescheduled at no additional charge.  

 - Cancellations made less than 24 hours before the scheduled appointment time will incur a non-refundable mobile service fee.  

 - Same-day cancellations and no-shows are non-refundable due to reserved appointment time and travel costs.

Unsuccessful Collections

• If a specimen collection cannot be completed due to client-related reasons, including but not limited to dehydration, refusal of service, unavailable or incorrect paperwork, improper preparation, or inability to safely complete the collection, the mobile service fee is non-refundable.  

• If a specimen collection cannot be completed due to an error on the part of West Phoenix Mobile Laboratory Services LLC, the client may choose one of the following options:

  – A full refund of the mobile service fee, or  

  – A complimentary rescheduled appointment.

Weather, Safety, or Emergency Delays

In cases of severe weather, unsafe conditions, medical emergencies, or circumstances beyond our control, West Phoenix Mobile Laboratory Services LLC reserves the right to reschedule the appointment. If rescheduling is not possible, a refund of the mobile service fee will be issued.

Laboratory Test Results

 West Phoenix Mobile Laboratory Services LLC is not responsible for laboratory test results, processing times, interpretations, or outcomes, as all testing is performed by third-party laboratories.  

 - Refunds will not be issued based on dissatisfaction with laboratory test results or laboratory processing issues.

Payment Disputes

 - Any payment disputes or refund requests must be submitted in writing within seven (7) days of the service date.  

 - Unauthorized chargebacks may result in denial of future services.
